Life and AD&D Benefits

Voluntary Life Insurance provides employees the opportunity to customize their individual life insurance needs.

Employee
• Coverage is available in $10,000 increments up to a maximum of $500,000.
• Minimum coverage: $10,000
• Guarantee issue: $100,000
• Benefits reduce to 50% at age 70

Spouse
• Coverage is available in $5,000 increments up to a maximum of $250,000; not to exceed 100% of the employee’s elected benefit amount;
• Minimum coverage: $5,000
• Guarantee issue: $25,000; (not to exceed 100% of employee amount)

Dependent
• Dependent coverage is only available if the employee is insured for Voluntary Coverage;
• Provides coverage for all dependent children up to age 26 in the following amounts: $1,000, $5,000 or $10,000 (not to exceed 100% of employee amount).

*Employees must enroll in coverage for themselves to enroll their spouse and/or children.
